Before Your Visit
Dr. Meyer requests that you download, fill out, and return via mail or fax the initial Intake Form prior to your first office visit so that your case may be reviewed and more time may be spent addressing your specific concerns at the time of your visit. Additionally, any recent laboratory work or diagnostic reports that you may be able to bring to the visit are very helpful.

What to Expect on Your First Visit
The first office visit generally consists of a comprehensive medical history taking and physical exam. The visit generally lasts about 90 minutes.
Insurance
Our office currently accepts BCBS, Cigna, MVP(Specialist) and VHAP-Vermont Medicaid. Other insurance plans may be accepted and billed out-of-network. If you are not currently insured or desire to pay for the appointments directly, we can provide you with a copy of our fee schedule upon request.
Cancellation Policy
Kindly give 24-hours notice if you will be unable to make your appointment.
